Trump administration freezes Title X family planning grants (April 2025)

April 2025 — The Trump administration abruptly withheld funding for 22 grants under Title X, the nation's only federal program dedicated to providing family planning services to low-income people, claiming that grantees were in potential violation of executive orders banning diversity, equity, and inclusion programs. Title X already explicitly prohibits funding for abortion care; the freeze targeted organizations solely because they also provide abortions separately, using non-federal funds.

Planned Parenthood affiliates were among the hardest hit. The administration's move echoed the first Trump term's "domestic gag rule", which had caused roughly a quarter of Title X clinics nationwide to withdraw from the program rather than comply with restrictions on abortion counseling and referrals. That rule had been rescinded under President Biden, prompting clinics including Maine Family Planning and Planned Parenthood affiliates to rejoin the program — only to face renewed defunding under the second administration.

Some frozen grants were eventually restored, but Title X funding for many Planned Parenthood affiliates remained withheld. Reproductive health advocates noted that the practical effect was to restrict access to contraception, STI testing, pregnancy testing, and primary preventive care for millions of low-income patients — services entirely distinct from the abortion care the administration cited as justification.
